Chronic Disease Fund is here to help patients that need assistance so that they can enjoy life without pinching pennies to pay for medication. The purpose of our reimbursement services is to make sure that the patients enrolled in our program enjoy the full benefits of copay assistance. We help in the following areas:

• Benefit verifications
• Prior authorization assistance
• Billing and coding support
• Appeals management
• Financial assistance coordination

Chronic Disease Fund assists patients throughout the United States who meet income qualification guidelines, or who have private insurance or a Medicare Part D plan, but cannot afford the cost of their specialty therapeutics. We pay our patients’ out-of-pocket costs on the spot and directly to their provider – without waiting periods or excessive paperwork. Also, if for some reason our patients spend their own money when picking up the prescription, we will reimburse them accordingly.
